The New Client Form is the starting point in our delivery of care. The initial information you provide flows through the website to Client Care. This team exists to make your entry into our practice as seamless as possible. They serve as a communication point with you in a host of areas including information-gathering and scheduling. If your New Client request is for Adult Individual, Marriage or Family therapy, it will be routed to Adult Client Care. If your request is for Child & Adolescent-related services, it will be routed to Child & Adolescent Client Care.
